Medical Records

If you were a patient prior to May 24, 2021 you will need to request your medical records directly from the hospital. Your information is secured and protected at OSH. To obtain a printed copy of your medical records, please download and complete the Authorization to Use or Disclose Health Information Form. Submit the completed form, along with a copy of your valid photo ID, via one of these methods.

Form Submission Methods

Patients who have signed up for the MyOSH patient portal can access details of their medical records by using the HealtheLife app. 

  1. Email recordsrequest@oksurg.com
  2. Fax it to the Medical Records office at 918-477-5050
  3. Bring it to the Medical Records office, located in our first floor lobby.
  4. Mail it to Oklahoma Surgical Hospital, ATTN: Medical Records, 2408 E 81st St S, Suite 300, Tulsa OK 74137
